WECS was formed in April 2015 and provides care to both Residential and Nursing Home residents and to housebound patients living in Weymouth and Portland.
The service is for patients who have an acute on the day need for a home visit who are frail, or becoming frail.
Patients with a variety of different issues including frailty, poor mobility, falls risk and risk of recurrent hospital admissions will be invited to this clinic for a holistic review of their needs.
The club provides leg ulcer management, advice, treatment of lower leg problems in a social environment.
A carer is a person of any age (including children) who provides unpaid support to a partner, relative, friend or neighbour who couldn’t cope without their help.
Healthy Living is a free online NHS service for people living with type 2 diabetes. It provides knowledge and information that will support you to manage your condition.
Spirometry is a basic breathing test which measures the amount of air you breathe in and out and the force you use.
